How we scaled professional spiritual care across 30+ dialysis clinics in multiple states—expanding access to professional chaplains for the first time to patients in outpatient dialysis clinics.

A national dialysis provider needed to meet the existential and spiritual needs of thousands of patients—without the resources to place professional healthcare chaplains in every clinic. These needs routinely surfaced in social work assessments: grief, identity loss, and decisions about palliative care or end of life. The organization also participates in value based reimbursement models that tie performance to key quality metrics—PHQ-9 depression scores, patient experience, and health equity.

To meet the needs of dialysis patients experiencing spiritual distress, SpirituWell delivers high-quality telechaplaincy via scheduled and on-demand phone and video visits. Each referred patient receives a PC-7 spiritual assessment and personalized care plan, with follow-up visits as needed. Referrals come through a streamlined workflow embedded into the provider’s social work protocols, and SpirituWell chaplains share back encounter documentation to support coordination of care.

Through this partnership, thousands of patients—many of whom previously had no access to spiritual care—are supported in ways that directly address the emotional and existential burdens of long-term illness. Additionally, Spanish-speaking chaplains help close longstanding equity gaps for patients where Spanish is their preferred language.
